Sports Day II will be held at the College of Staten Island Sports and Recreation Center.

Doors Open at 8:00 AM
Cost: $5 for all spectators and coaches

To all PYFA members interested in participating in the 2009 Sports Day Events, here's what you need to know...

Below you will find the downloadable registration forms and tournament guidelines for this year’s Men's, Women's and Junior's Basketball and Volleyball Tournament's. Please review all attached materials and dates.

In order to be eligible to play the following must be completed:

1. Complete Registration form with Pastor & Youth Director’s signature.

• Registration forms will ONLY be accepted beginning May 15th (after Take Back the Night 2) and ending June 7th at 6:30 pm (after the Committee Meeting). NO FORMS AND PAYMENT WILL BE ACCEPTED AFTER THAT DATE AND TIME!
• Hand in separate forms for teams with players from different churches playing on one team.
• T-shirt Color Preference is determined by the date and time the registration form is received.

2. If anyone on a team is under the age of 18 a Parental Waiver form must be signed and attached to the registration form.
3. All players must submit a copy of their photo ID.
4. Registration fee must be paid in full.
5. Church Membership Dues must be paid on or before June 7th.
